I created a switchboard using switchboard manager and accidentally deleted
the "switchboard" icon from my Forms objects.  I can't run the switchboard
even though the info (structure) is all there.  How do I get the
"Switchboard" icon back in my forms objects?

Followed the help file instructions for setting a database password. Now I
can't open any database at all. I am stopped at the user/password box. I
