I suspect you do not realize that you have posted your request to a
newsgroup whose subject is Questions and Answers about Microsoft Access
database software.

If you are asking for someone to create such a Form in Access, that is not a
usual "service" of this newsgroup, but someone might volunteer to do so, if
you provide the database in which you want the Form to reside.
