I have a data base that opens directly into a FORM that looks similar to -
but is not a switchboard.  It requires that you hold down the shift key, when
clicking to open the folder to access the design view and do any build.  How
did she set this up?

On the menu go to Options  >> Startup
this is where the database knows what to do when it starts up
under Display Form/Page you will find the name of that switchboard - like
form that opens first.
If you change the form name to (none) the and mark with a cheque the option
called Display Database Window, you can open the database without using the
Shift key. To do this you will need to close the database with the new
settings saved and open it again to see the effect of the new settings.
