Open the Report in design view, and select File... Page Setup from the menu.
Specify the paper size there (and the printer, and landscape/portrait, and
color, and any other specifics); close the window and save the Report. It'll
hold those settings for the future.

Custom is not an option in the Size list box. It has predefined sizes such
as Letter, Envelope, Legal, A6, etc, but none of these will work for me. I'm
trying to print Jukebox Title cards. Any other suggestions? - than ks -

Try creating a new printer definition with the custom paper size and using
that new definition.

That's a function of your operating system, not of Access. Go to the Printer
and Faxes dialog and add a new printer.
