Access DB template for customer problem resolution?

We would like to have a triage database for problems that we encounter with
different problem situations and their resolution.  Are there any templates
available that might fit a similar format?

Check the Service Call Management Database.  It might get you started.

Thanks!  I didn't really want to cost parts, but it is a good template.
