database tables creation

I am trying to build a database in Access and am new to this software . Can
someone start me on this
Its a feedback database. Customers submit their feedback form and we get
data from that.
Form contains  suggestion, compliments , complaints, requesting information
check boxes and feedback notes.customers  can fill several feedback  forms
for various not for profits agencies within the hub either by staff employee
name or/and agency name
Customers name and telephone  is optional.
What tables do I need and what kind of relationship do they have with each
other to have normalized form.

Have you taken a look at the templates Microsoft offers?  You can go on-line
to microsoft.com, type in "templates" for a search term, and review the
starter sets Microsoft offers.
