I have a data base that opens directly into a FORM that looks similar to -
but is not a switchboard.  It requires that you hold down the shift key, when
clicking to open the folder to access the design view and do any build.  How
did she set this up?

Tools, Startup has a dropdown to specify the form to open on startup.

(it's also possible this was done in an autoexec macro).

Holding the shiftkey down will bypass the settings in tools, startup and also the autoexec macro.  The shiftkey bypass can be disabled.
