So many people can add data to it working from both PCs and macs, I already
made the database, it works as I want it to, 3 levels of one to many
relationships, forms, queries to make calculations used in the forms, but I
am no programmer (I don't count my BASIC training when I was 11), went on to
become an MD and am at a real loss as to how to get this to work, since the
proyected data will be from around 6000 patients, each with 1 to 4 surgeries,
and each surgery with 10 to 20 consultations and exams related...

Dear Sir,
This is probably not a good project for you to develop yourself. If any of
the data you are displaying/collecting involves personal health information
you must be very rigorous in applying security to the website, etc. HIPAA
and privacy laws both apply in these situations (at least in the United
States).
