Hi, I'm new at access. I was wondering how do u recreate a form
(questionaire) into access. Then creating a database of this form so that
agent can input info from the questionaire sheet into the data base. By doing
this we have a log of people that have came to see our property. So if
someone were to come visit again we would have a log of what they wrote on
the questionaire.

From your questions, it sounds as if you might be more comfortable doing
this in Excel.

Broadly speaking, you create the db, then the form.
Sketch out your requirements on paper first.
Perhaps buy an Access book?
