I created a switchboard using switchboard manager and accidentally deleted
the "switchboard" icon from my Forms objects.  I can't run the switchboard
even though the info (structure) is all there.  How do I get the
"Switchboard" icon back in my forms objects?

Try to create a new one with different name - save.  Then rename it.

* Rename the "Switchboard Items" Table to a temporary name like "tblTemp".
* Run the Switchboard Manager again.  Since the Manager can't find anything,
it will re-create both the Form and the Table "Switchboard Items" again for
you.
* Close the Switchboard.
* Delete the new "Switchboard Items" Table (but NOT the Form).
* Rename "tblTemp" to "Switchboard Items".
