duplicate relationships

I want to generate a form, to be used as a data entry tool, to enter the
company/client information without having to enter through each table.  When
I created the form and chose the tables for the query.  I get an error.  When
I check the relationships I now have duplicate tables appearing.  Any advice
on what I did wrong?

What is the error you got?

Often duplicate tables in the relationships window are not a problem, but if
they bother you, you can delete those relationships.

Nevermind...I went back to try again to see exactly what the error was - but
I really messed up somehow.  I have a new message: MS Access can't find the
database file.  I think I need to start over.
