Setting up a Data Entry Form

Okay,
I keep running into problems on this database here and there. The problem is
that I have a table with Employee Number, Date, Job Type, Training code,
Pass/Fail checkbox, Hours, Minutes. I would like to create a form that lets
me enter new records using combo boxes for Training Code, and Job Type. How
would I set the form up so that it saves new data into the existing table
from the form. I understand that it will save for me, but how do I set up the
form (fields to be blank, etc.)

If you set the Data Entry property to Yes the form will open to a new record.
