I need a form to have fields that autofill based on an input.

I am trying to have a form prefill 3 fields based upon one input on the table.

pretty vague question.

If you mean that you want to enter a client number and have their name and
address display (for example) then look at the Northwind database "orders"
form and see how it works.  You can copy that functionality.
