I have a document set-up in word that requires information input onto a line,
eg. Name: ______________________. How do I format the line so when someone
opens the form in word, they can just type the answer onto the line?

You are looking for some kind of input field (maybe ASK), but you will get a
much better response if you will post your message in one of the Word
newsgroups. This newsgroup is for MS Access, the database product.

I'll answer it here for quickness sake. I would use a two column table.
Draw all the lines with No Border except the line under the entry.
Open the Forms toolbar and insert a text field and get rid of the
shading. Protect it as a fill in form and the user will be able to
complete only those form fields, not the labels. Hope this answers
