Custom Menu Item does not show up in distribution

I'm developing in Access 2003 and packaging with Visual Studio Tools for MS
Office.

I've added a menu item to the default menu in Access but it doesn't show up
in the run-time distribution.
What am I doing wrong?

Customizations to built in menus stay on your machine and are seen in ALL
files you open.  If you build *totally new* menus and toolbars they stay
with the file and will be seen on other machines and on your machine will
only appear in that one file.
