How do you create the MDB or MDE as a runtime version? Thought you needed to
create an install package to create a runtime version?

The runtime is a crippled install of *Access* itself. Once it is installed
with your original installation program you do not need to install it again.
All you need to do is replace your file with the new version and the runtime
that is already there will run it.
